Appointments

All new patients will be scheduled with our hygiene staff for an initial oral evaluation. If you have recent x-rays or records, please bring them with you to your first visit if possible. We do have release forms available for your use if necessary. In a case of a dental emergency, we will make every effort to schedule you that same day during regularly scheduled office hours. In a case of a serious dental emergency, the office voice mail is linked to Dr. Hogan’s cell phone. Patients of record may be seen outside of normal office hours in these emergency situations.

Insurance and Billing

While not a participating office in any DMO or PPO, we gladly process and bill your insurance carrier for treatment provided by our office. We are in constant contact with insurance carriers and by our direct, hands-on approach; we take this burden off of you. Patients or their guarantor are responsible for the balance not covered by insurance. Your insurance benefits are based on an agreement or contract between your employer and the insurance carrier, not by any dental practice. Patients are financially responsible for their care, regardless of insurance benefits.

Payment Options

We accept checks, cash and credit cards for payment of dental treatment. Access to the program, Care Credit, may be available to qualified individuals. While we will accept assignment of insurance benefits, patients or their guarantors are ultimately responsible for payment for dental care received.

Facilities and Equipment

The Park Ridge Medical Plaza is an accessible building featuring Disabled Parking in the front driveway, automatic front and rear entrance doors, wide halls and doorways to accommodate most double-size and powered assisted wheel chairs.
